A team member recovers a crumpled Oxy-generator from the Florida Everglades. This evidence will help determine what exactly caused ValuJet Flight 592 to crash minutes after taking off from Miami International Airport with 110 passengers and crew on board. The oxygen generators are installed in passenger planes to provide emergency oxygen. They produce a chemical reaction that produces instense heat. Investigators wonder if they somehow contributed to the crash.

Local divers step out of the Musi River with a fishing net full of plane debris from Silkair Flight 185, including the Flight Data Recorder that may give investigators some insight on why the Boeing 737 crashed on December 19, 1997 killing all 104 people on board. But the recorder will only deepen the mystery. Investigators will discover that thte recorder cut out just before Flight 185 went into a steep dive. The find will lead investigators to consider a troubling possibility; someone may have deliberately disconnected the Data Recorder.

Investigators from Taiwan's Aviation Safety Council must now piece together the clues to determine how the crew could have made such a deadly mistake.

The data should explain why the commercial jet, carrying 181 passengers and 8 crew crashed into a wooded area 6 miles short of Portland International Airport. REENACTMENT - AAIB technician (played by an actor) examines an attitude indicator recovered from the wreckage of Manx2 Flight 7100. The commuter plane crashed at Cork Airport in southern Ireland on February 10th, 2011, killing 6 and badly injuring 6 others.
NTSB investigators (played by background actors) document the wreckage from John F. Kennedy Jr.'s Piper Saratoga looking for clues to explain why the plane crashed, killing Kennedy, his wife Carolyn Bessette-Kennedy and his sister-in-law Lauren Bessette.
Runway 5R at Taiwan's Chiang Kai-Shek International Airport has been closed for repairs for months. Somehow a Boeing 747 collided with the heavy equipment being used for the repairs on the night of October 31, 2000. Investigator Kae Yong (played by Frank Nakashima) from Taiwan's Aviation Safety Council inspects the wreckage to help understand how the passenger jet, with 179 people on board collided with the construction equipment, killing 83 people.
Investigators study the debris collected from Silkair Flight 185, as divers prepare to search the river for blackboxes after the plane nosedived from 35,000 feet to the ground on December 19, 1997. Without the black boxes investigators may never be able to explain why the Boeing 737 suddenly went into a steep descent, killing all 104 people on board.
