Larry Flynt und sein Bruder sind als Betreiber eines Stripclubs in Ohio der bitteren Armut ihrer Kindheit in Kentucky entkommen. Das Geschäft läuft trotz der sexuellen Revolution der 1970er Jahre nur mäßig; aber Larry, der bestrebt ist, alle seine Tänzerinnen ins Bett zu kriegen, ist in Geschäftsdingen auch nicht überambitioniert. Um Kunden in seinen Club zu holen, lässt er in lokalen Zeitungen Anzeigen mit nackten Frauen drucken.Das bringt ihm zwar reichlich Ärger mit verschiedenen Moralaposteln ein, aber so kommt er auch auf die Geschäftsidee seines Lebens: Er gründet das Pornoheft "Hustler", das alles zeigt, wofür sich der "Playboy" zu fein ist. Der Mut zum Vulgären und Provokanten macht den "Hustler" zu einer der erfolgreichsten Publikationen in seinem Segment. Doch die religiöse Rechte schießt sich immer mehr auf das Magazin ein; umso heftiger, als Flynt sie im "Hustler" mit Hingabe bloßstellt und attackiert.So beginnt eine nicht enden wollende Kette von juristischen Auseinandersetzungen, die sein Anwalt Alan Isaacman für ihn durchfechten muss. Doch Flynt will nicht klein beigeben.

Auch nicht, als ein religiös verblendeter Attentäter ihn anschießt und er daraufhin an den Rollstuhl gefesselt ist; und auch nicht, als seine Frau Althea immer mehr den Drogen verfällt und sich mit HIV infiziert. Flynt geht weiter durch alle juristischen Instanzen, klagt sich bis zum Supreme Court durch und wird so zum Helden im Kampf um die Meinungs- und Pressefreiheit.

Der Film spielt in den Jahren 1952, 1973, 1975, 1978, 1986 und 1987 in Larry Flynts Leben. Ashley Judd, Cameron Diaz, Patricia Arquette, Mira Srovino und Kimberly Williams-Paisley waren im Gespräch für die Rolle der Althea Leasure, der Stripperin, Freundin und späteren Frau Larry Flints. Williams-Paisley stieg von sich aus wegen der geforderten Nacktszenen aus. Courtney Love war auf Drogen, als sie für die Rolle vorsprach, doch nach anfänglichen Zweifeln war sich Regisseur Miloš Forman sicher, dass sie die richtige Besetzung für die verdrehte Althea sei. Bedingung war: Love, die selbst einmal als Stripperin gearbeitet hatte, musste während der Dreharbeiten sauber sein. Sie hielt sich dran. Edward Norton übernahm die Figur des Anwalts Alan Isaacman übrigens, weil er unbedingt einmal mit Forman arbeiten wollte. Achtung, Cameo-Ironie: "Hustler"-Verleger Larry Flynt spielt einen Richter, der den Larry Flynt im Film (Woody Harrelson) ins Gefängnis bringt. Der Medienmann wurde, obwohl seine Person die Hauptfigur des Streifens ist, zur Oscar-Verleihung explizit nicht eingeladen. Er kam trotzdem: Hauptdarsteller Woody Harrelson nahm ihn als seine Begleitung mit.
