Johnny Sledley, genannt "der Schöne" ist alles andere als schön: Seit seiner Geburt ist sein Gesicht entstellt. Er spricht nur mühsam und hat es nicht weiter als zum kleinen Gauner gebracht. Als sein Freund Mikey ihn darum bittet, sich an einem Überfall zu beteiligen, ist Johnny zwar skeptisch, willigt aber ein. Der Überfall auf eine Münzhandlung läuft jedoch nicht nach Plan. Der Geschäftsinhaber wird erschossen, und das Gangsterpärchen Rafe und Sunny tötet auch Mikey, um die Beute nicht teilen zu müssen. Johnny zahlt allein die Rechnung: Er wird zu fünf Jahren verschärfter Zwangsarbeit verurteilt.

Obwohl ihm eine mildere Strafe in Aussicht gestellt wird, verrät er seine Komplizen nicht. Er will den Tod seines Freundes persönlich rächen. Nachdem Johnny in Rafes Auftrag im Gefängnis überfallen wurde, wird er schwer verletzt in ein Krankenhaus in New Orleans verlegt. Dort behandelt Dr. Resher nicht nur Johnnys Stichwunden. Er macht seinem Patienten im Rahmen eines speziellen Resozialisierungsprogrammes einen gewagten Vorschlag: Mit einer kosmetischen Operation und einer neuen Identität könnte Johnny ein völlig neues Leben beginnen.

Der willigt ein. Nach wochenlangen Operationen und Schmerzen sieht er im Spiegel ein neues Gesicht und erhält seine zweite Chance. Johnny wird auf Bewährung freigelassen. Er bekommt einen Job bei einer Werft und freundet sich dort mit der Buchhalterin Donna an. Aber immer noch beherrscht ihn ein Gedanke: Rache für Mikey. Er bietet Rafe und Sunny ein Fünf-Millionen-Dollar-Geschäft an: Einen Überfall auf die Lohnkasse der Werft.
