Auf der Suche nach einem - inzwischen an Krebs gestorbenen - Kameraden aus dem Vietnamkrieg verschlägt es John Rambo (Sylvester Stallone) in ein kleines Provinznest. Der übereifrige Polizeichef des Kaffs, Sheriff Teasle (Brian Dennehy), gabelt Rambo auf und deportiert den vermeintlichen Landstreicher mit den Worten: «Solche Typen wie dich wollen wir hier nicht.» Rambo fühlt sich ungerecht behandelt und kehrt hinter dem Rücken Teasles wieder zurück, nur um erneut aufgegriffen zu werden.
Doch diesmal lassen es der Sheriff und seine Helfer nicht bei einem Verweis bewenden. Genüsslich schikanieren und demütigen sie ihren «Gefangenen», bis diesem der Geduldsfaden reisst. Rambo rastet aus und setzt sich mit einem gestohlenen Motorrad in den nahe gelegenen Wald ab. Teasle, angestachelt durch das renitente Verhalten Rambos, ruft zu einer Menschenjagd auf, obschon die aus dem Computer abgerufene Information wenig Erfreuliches zutage födert: John Rambo wurde mit der Congressional Medal of Honor ausgezeichnet und ist nicht nur ein hochdekorierter Vietnamheld, sondern auch in der Guerillataktik geschult. Und so kommt es als Folge von Vorurteilen und Kommunikationsunfähigkeit im US-amerikanischen Hinterland zu einem zweiten Dschungelkrieg.
Nach der Box-Ikone Rocky schuf der oscarnominierte Sylvester Stallone mit dem missverstandenen Vietnamveteranen Rambo eine weitere Figur von symbolhafter Stärke. Während der engagierte Regisseur Ted Kotcheff in «Rambo - First Blood» - wenn auch auf unzimperliche Art - Themen wie Kriegstrauma und Neurosen aufgreift, gehen solche Differenzierungen den später folgenden und für den «schlechten Ruf» Rambos verantwortlichen Fortsetzungen ab. Stallone verkörpert den mental herausgeforderten Vietnamkämpfer mit der gleichen Überzeugungkraft wie einst den boxenden Underdog Rocky Balboa.
In den letzten paar Jahren hat er die beiden Kultfiguren in «Rocky Balboa» (2006) und «Rambo» (2008) wieder aufleben lassen. Am Zurich Film Festival 2008 war Sylvester Stallone Ehrengast. «... in den von Paranoia und mörderischer Angst (auf beiden Seiten) geprägten Nachtszenen zeigt sich der Kanadier Ted Kotcheff als brillanter Regisseur», schrieb Die Zeit über «First Blood», diesen umstrittenen Klassikers der neueren Filmgeschichte.

Als Rocky Balboa und John Rambo ging Sylvester Enzio Stallone in die Filmgeschichte ein und avancierte zu einem der erfolgreichsten Darsteller Hollywoods. Doch bevor "Sly" als Action-Held gefeiert wurde, verdingte er sich in drittklassigen Produktionen und übernahm kleinste Nebenrollen. Erst eine Reportage über Muhammed Ali brachte die Wende: Sie inspirierte ihn zum "Rocky"-Drehbuch. Der Boxer-Klassiker machte ihn über Nacht zum Superstar und brachte ihm 1977 zwei Oscar-Nominierungen ein. Seitdem schoss und schlug sich Stallone erfolgreich durch Produktionen wie "Demolition Man", "Cliffhanger" und "Assassins". Nachdem es eine Weile ruhig um den Actionstar war, meldete er sich 2007 mit "Rocky Balboa" und "John Rambo" erfolgreich zurück. Im Herbst 2010 lief "The Expendables" erfolgreich in den deutschen Kinos, in dem Stallone als Regisseur und Hauptdarsteller den Actionfilm der 80er-Jahre wieder aufleben ließ. Zwei nicht weniger erfolgreiche Fortsetzungen folgten 2012 und 2014. Richard Crenna ("Sabrina"), der in allen "Rambo"-Filmen John Rambos Ausbilder Colonel Samuel Trautman spielt, starb am 17. Januar 2003 an einem Krebsleiden. Regisseur Ted Kotcheff setzte in seiner Verfilmung von David Morells Roman vor allem auf publikumswirksame Knalleffekte. Trotz der Bedenken Kotcheffs bestand Stallone darauf, die meisten der Stunts selbst auszuführen. Bei der Szene, in der er von einer Klippe springen musste, brach er sich denn auch drei Rippen.
